Documents to be submitted
- Birth Certificate.
- NIC or valid passport or valid Sri Lankan driving license.
- Print out of the application with your signature with certifying as per the paragraph no.7.3
- G.C.E. (A/L) results sheet with the Z score. (Getting pass the 3 subjects at one sitting of the Science or Mathematics stream of year 2019 (Old syllabus) or (New syllabus) or getting pass the 03 subjects at one sitting of the year 2020 from the Old syllabus or New syllabus issued only by the Examination Department (Except General knowledge and English)
- G.C.E.(O/L) results sheet ( Issued only by the Examination Department)
- Receipt obtained for the payment of Rs. 1,000.00
- Grama Niladhari Certificate (DS 4 form)
- Student Performance Sheet (School Leaving Certificate - Education B 59 form)
- Affidavit signed over a stamp at the value of Rs. 50.00 in the presence of a Justice of Peace to confirm that the applicant is not married or divorced (Annexture 3)
- Solemn Declaration to confirm the applicant is not a registered student or currently following full time / internal degree programme conducted by a State University or a State University College recognized by the University Grant Commission "under the national free education policy." (Should be completed and submitted the Solemn Declaration which is posted on the website) (Annexture 4)
- If there is name change in the above documents, need an affidavit from the Justice of Peace which is duly signed on the stamp of Rs. 50.00
