1. Enter your NIC number and District. Than Click DISPLAY 
2. Click PRINT POSTAL VOTE APPLICATION (you will get the PDF)

Click Display and you will get this

Then Enter Mobile Number , and Captcha details , then  Click Print Postal Vote Application.

You will get a pdf, take a printout and fill the rest.
